Our model
Collaborative OB/Midwifery care, supported by doulas and specialists
Certified Nurse Midwives
The primary clinicians for gynecology and maternity at Millie, with OB-GYNs co-managing care when more is needed. Trained and licensed as both registered nurses and midwives. Known for their intimate knowledge of pregnancy and labor and their minimal-intervention approach.
Extensive research shows that CNMs are the ideal care providers for most pregnancies. When midwives are involved in pregnancy care, there are fewer unnecessary medical interventions, higher rates of vaginal births after cesarean section, and higher patient satisfaction.
OB-GYNs
Doctors and surgeons trained to perform cesarean sections, vacuum or forcep deliveries, and other procedures in more complex pregnancies. Some OBs further specialize to become Maternal Fetal Medicine (MFM) specialists or perinatologists.
At Millie, clinic and hospital-based OB-GYNs co-manage care alongside Millie midwives when additional expertise is needed to manage your pregnancy.
Doulas
Professionals trained to provide educational and emotional support throughout the experience of having a baby. Every patient at Millie is paired with a Millie Guide, a practicing doula who meets with you each trimester to go over key topics like creating birth preferences and postpartum planning.
Your Millie Guide is also available to answer questions that come up between visits through the Millie app.

What is a midwife?
There are many types of midwives, but at Millie, we employ Certified Nurse Midwives (CNMs). CNMs are medical experts who are trained and licensed as both Registered Nurses and Midwives. They are known for their intimate knowledge of pregnancy and the labor process, and their minimal-intervention approach that promotes physiological birth.
What is a midwife vs. an OB, and why do both matter?
Millie employs Certified Nurse Midwives (CNMs) — licensed as both Registered Nurses and Midwives — known for a minimal-intervention approach that promotes physiological birth and meets about 80% of clinical needs during pregnancy and childbirth. OB-GYNs are medical doctors and surgeons trained to perform procedures like cesarean delivery and gynecologic surgery, and are the ideal clinician for higher level of care; some further specialize as Maternal Fetal Medicine (MFM) specialists. Both share the same goal of healthy pregnancies, safe births, and healthy babies, and how much of each type of care is needed depends on the pregnancy.
What does collaborative OB/midwifery care look like at Millie?
Millie believes everyone needs a midwife, some people need an OB-GYN too, and everyone should have access to both. Patients establish care with a CNM of their choosing, who serves as primary clinician. Higher-risk pregnancies are co-managed with the hospitalist OB-GYNs at Good Samaritan Hospital. Some pregnancies may become too high-risk for care at Millie, and in these instances we will work with you to transfer care to an OB-GYN in your community with a seamless transition of care.

Millie midwives currently staff Labor & Delivery at the hospital Monday to Friday, 9am-5pm with the hospitalist OB-GYNs providing care after hours and on weekends in close collaboration with the Millie team. We are working on 24/7 coverage by the Millie midwives in the coming months.
Does Millie follow the same care protocols as standard OB practices?
Yes – we follow the same care protocols in terms of visits, labs, imaging and other testing as standard OB care and we are equally committed to your safety and the baby’s health. But we go further – with more hands-on support on labor & delivery, robust postpartum care (3x visits, including one at home), educational resources, and wraparound support.
Midwives, who lead care at Millie, are known for their holistic approach to pregnancy care and expertise in physiologic birth outcomes. When more is needed, clinic and hospital-based OBs co-manage care alongside midwives to deliver healthy outcomes for you and baby.
Is my pregnancy a good fit for Millie?
An estimated 80% of pregnancies are low-risk and another 10-15% are moderate-risk; Millie care is designed for this population. Millie does not accept high-risk pregnancies at this time, but we are happy to make a referral. Clinical eligibility details are available for the South Bay clinic.
Who will be at my birth and deliver my baby?
Millie midwives and the OB-GYNs at Good Samaritan work together, sharing clinical protocols and a common patient record for a seamless transition from pregnancy to delivery and postpartum care. If a c-section or other intervention is needed, the hospitalist OB-GYNs perform it in collaboration with the Millie midwives.

Can I get an epidural if I’m a Millie patient?
Yes - you have access to all the same pain relief options - including epidurals - that you would in a standard OB care setting. Your care team will help you understand your choices and support whatever birth plan works best for you.
Do you take insurance? Which plans?
Millie currently accepts PPO plans from Aetna, Cigna, United Healthcare, Anthem Blue Cross, Blue Shield of California, and HealthNet, as well as some local networks. Millie accepts HMO plans via SCIPPA. Millie also accepts some Medi-Cal plans via NEMS.

What will I pay for my care?
All clinical appointments, lab tests, ultrasounds, and the labor and delivery experience at Alta Bates Hospital are billed to insurance. In-network lactation services are also offered and accepted by most major insurance plans, including Medicaid. There is no extra charge for the Millie app, remote monitoring devices (e.g., blood pressure cuff), or group sessions and messaging with a Millie Guide (a practice-embedded doula). Consultations for supportive care such as educational classes, workshops, postpartum groups, therapy, and nutrition are not currently covered by insurance and are paid out of pocket (may be HSA/FSA eligible).
